You Need An Appointment Control Number
You need an Appointment Control Number (ACN) before most civilian ADDP visits. If a military dental clinic referred you, the ACN is on the referral. If you are remote, get the ACN from United Concordia first, then book with us.

Dr. Paulo F. Cortes enlisted in the Air Force at seventeen. He served as a diet therapist at Elmendorf Air Force Base in Alaska and separated as an Airman First Class.
He joined the United Concordia Active Duty Dental Program to give busy base dental clinics some relief, so service members do not slip through the cracks. Those clinics work hard. They are busy. They refer patients here when a civilian visit is the right next step.
He wants the same thorough care for you that he would want if he still wore the uniform.

Who ADDPIs For
ADDP is for active duty service members who need civilian dental care. It is not a family plan, and it is not a list of civilian PPOs.
- Active duty service members referred from a military dental clinic, also called a dental treatment facility (DTF).
- Remote active duty service members who live and work more than 50 miles from a military dental clinic.
- Not the TRICARE Dental Program (TDP). TDP is the family dental plan. This page is only for ADDP.
How ToCome In
Get the ACN first, then hold a Friday or Saturday here. We will not guess at coverage without that number.

Get your ACN.
Use the ACN on your DTF referral, or get one from United Concordia online or at the CONUS line, 866-984-2337.

Call or text Kearny Mesa Smile Center.
Call or text (858) 256-4838 any day. Tell us you are coming under ADDP and that you have your ACN.

Come Friday or Saturday.
This office is open Friday and Saturday, 8:00 AM to 2:00 PM, at 8018 Clairemont Mesa Blvd, San Diego, CA 92111.

Bring the ACN.
Bring the Appointment Control Number with you. If it is on a printed referral, bring that too.

If you are a remote active duty service member, care for pain, infection, or bleeding may not need an ACN first. Call or text us and say what is going on.
Crowns, bridges, and dentures are not emergency care. If those are the only services you need, get an ACN first.
