When A DentureIs Not Enough
A conventional denture rests on the gums, which means it moves. People adapt by chewing carefully, avoiding certain foods, and quietly editing what they order in restaurants. Many stop wearing the lower one altogether.
Underneath, the bone continues to shrink, because nothing is stimulating it. Over years the ridge the denture sits on gets flatter, and a denture that fit at the start fits worse and worse.
Fixed teeth on implants address both. The implants hold the teeth still, and because they are anchored in the bone, they give that bone something to do again.
How All on X Works
01. Records. A CBCT scan, a digital intraoral scan, and photographs. Together these give us your bone in three dimensions, the shape of your mouth, and how everything sits relative to your face.
02. Planning, with a lab. Those records go to a dental laboratory that specializes in this work, and Dr. Cortes builds the treatment plan with them. Where each implant goes, at what angle, at what depth, and how the final teeth will sit on top of them.
03. The surgery, performed on screen first. The whole procedure is carried out in CAD/CAM software before anyone touches a patient. Every position is worked out and checked while it is still only a file.
04. Custom surgical guides. Dr. Cortes then fabricates guides made specifically for your mouth, in this office. They fit over your jaw and physically direct each implant to the position that was planned.
05. The surgery. Dr. Cortes places the implants at this office, using those guides.
06. Healing. The implants integrate with the bone over the following months.
07. The final teeth. Once everything is solid, the final fixed arch is made and fitted.
The Planning Takes The Time.The Smile Does Not.
You have probably seen advertising promising teeth in a day. Here is what is actually true.
The work that takes time is the planning. The scans, the software, the positions, the guide. That happens before you ever sit down for surgery. Once it is done and the case is planned properly, many patients leave on the day of surgery with a fixed set of teeth already in place. Not something you take out at night. Teeth that are attached.
Your final set comes later, after the implants have fully integrated with the bone. But you are not walking around without teeth while that happens, and you are not waiting months to see what this is going to look like.

If you want access to everything on the table, a full arch fixed on implants is what gets you there. It does not move, it does not come out, and you bite with it the way you bit with your own teeth.
Below that, the percentage starts dropping. A snap-in denture over implants gets you to roughly seventy percent of what you could eat before. A traditional denture sits closer to half.

Why guided surgery matters

This is the part worth understanding, because it is the difference between a planned surgery and a judged one.
The entire procedure is performed in software first, using a 3D scan of your actual jaw. Implant positions, angles and depths are all decided and verified there, where changing your mind costs nothing. Dr. Cortes then fabricates physical guides from that plan, in this office, which sit over your jaw during surgery and put each implant exactly where it was placed on the screen.
So when Dr. Cortes performs your surgery, it is the second time he has performed it. The first time was on the computer. That is what makes it safer and more predictable, and it is why we do every implant placement this way rather than only the complicated ones.

This is for people who have lost most or all of the teeth in an arch, or who are about to, and who want them replaced with something fixed rather than something removable.
It works best when there is enough bone to anchor the implants, which the CBCT scan tells us before anything is planned. When there is not enough, grafting can build the site up first. Either way you will know where you stand at the planning visit, not partway through treatment.

Asked to put it on a scale of one to ten, with ten being the worst pain they can imagine, most report somewhere between a two and a four. You will be given specific aftercare instructions and something for discomfort before you leave.

We would generally rather place more than fewer, because more implants means more anchorage for the arch that sits on them. The fee is the same whether your case takes four or six, so the number is decided by what is best for your mouth and not by cost.

Dr. Cortes performs this surgery at this office. For some cases we bring in a specialist to work alongside him here, and for others we refer that portion of the treatment to the specialist's own office. You will know which applies to you before treatment starts.
